  [摘要] 目的 探讨多普勒组织成像在高血压患者二尖瓣环运动对左室收缩功能的作用。方法 以多普勒组织成像(DTI)技术检测心尖四腔心切面二尖瓣前叶瓣环与后叶瓣环的收缩运动速度S1、S2,比较正常人与高血压病患者心功能Ⅰ级、Ⅱ级、Ⅲ~Ⅳ级间的差别,以及高血压病患者不同EF间S1、S2的差别,并计算EF与S1、S2的相关性。同时比较各组中S1与S2之间的差别。结果 正常人与高血压病心功能Ⅰ级组间,S1、S2无差别,高血压病心功能Ⅰ级组与Ⅱ级组间亦无差别,但心功能Ⅱ级组较正常人降低,Ⅲ~Ⅳ级组较Ⅰ级组、Ⅱ级组更低。依EF不同分组后,EF越高,S1、S2越高,两者呈正相关关系,r分别为0.52,0.56,P<0.001。各组中S1、S2之间未见明显差别。结论 DTI技术检测二尖瓣环运动速度可以反映左心收缩功能的改变。对于没有节段性运动异常的患者,前叶瓣环、后叶瓣环都可以作为左心室收缩功能的代表,是一种无创检测左心收缩功能的方法。

Observation on mitral annular movement with Doppler tissue imaging in hypertensive patients
[Abstract] Objective To find an effect that left ventricular systolic function in hypertensive patients by mitral annular movement with Doppler tissue imaging.Methods Anterior and posterior mitral annular velocity(S1,S2) in systole from apex four chamber view were measured by Doppler tissue imaging(DTI).S1,S2 in healthy patterns were compared with patients with hypertension in different NYHA.We also compared S1 and S2 in different EF and analysis relative coefficient between EF and S1,S2.Results There is no difference in S1,S2 between normal patterns and hypertensive patients in NYHA Ⅰ,hypertensive patients in NYHA Ⅰ and Ⅱ.That of patients in NYHA Ⅱ were lower than normal patterns,in NYHA Ⅲ~Ⅳ were lower than both in NYHA Ⅰ and Ⅱ.Grouped with different EF,patterns were found S1 and S2 changed with EF positively(r=0.52,0.56 respectively,P<0.001).In all groups,no difference were found between S1 and S2.Conclusion Detection of mitral annular velocity with DTI can reflect the change of left ventricular systolic function.To patients without abnormal regional wall motion,either S1 or S2 can represent left ventricular systolic function.It is a method for assessment of left ventricular function.

多普勒组织成像(Doppler tissue imaging,DTI)技术是新近发展起来的一项多普勒诊断技术[1],不同于传统的血流多普勒。它以低频滤波器弃去高频率、低振幅的血流信号,专门检测心肌组织反射回来的低频率、高振幅频移信号,经彩色编码后,以室壁心肌的多普勒频移彩色图像显示。同时它的`脉冲速度频谱模式(DT-PW)能显示取样容积的运动速度和方向,是定量心肌运动的一种新方法。本文旨以该方法测定二尖瓣环的收缩期运动速度,比较它在不同心功能期的差异,以探求无创检测左室收缩功能的方法。
1 资料与方法
1.1 一般资料 选择正常健康志愿者21例,男13例,女8例,平均年龄(42.5±11.4)岁。高血压病患者45例,男28例,女17例,平均年龄(54.7±12.9)岁,纽约心功能分级Ⅰ级10例,Ⅱ级20例,Ⅲ~Ⅳ级15例。
1.2 方法 采用日立EUB-6500电脑声像仪,经胸变频探头2.5~4.0 MHz。受检者左侧卧位,二维超声心动图取左心长轴切面,M型取样线置于二尖瓣腱索水平,取得左室的M型曲线,分别测量左室舒张末期内径、收缩末期内径,计算左室舒张末期、收缩末期容积,并计算左室EF值。取心尖四腔心切面,在DTI模式下,以DT-PW将取样容积置于二尖瓣后叶、前叶瓣环,记录该频谱曲线,分别测量两处收缩期速度,取3~5个心动周期,平均值计为S1、S2。
