等离子治疗肥厚性鼻炎、变应性鼻炎疗效观察

【摘要】 目的 探讨等离子治疗慢性肥厚性鼻炎、变应性鼻炎的`临床疗效。方法 回顾2005年1~8月来我院就医，明确诊断为慢性肥厚性鼻炎899例，变应性鼻炎513例，通过术后经电话随访、患者来院复诊记录等进行临床资料的统计。结果 慢性肥厚性鼻炎治疗899例，痊愈620例，好转242例，无效37例，有效率95.88%；变应性鼻炎治疗513例，痊愈325例，好转160例，无效28例，有效率94.54%，疗效显著。结论 等离子技术应用于临床的时间不长，其与微波、高频电刀、激光光导纤维等设备的不同是超低温技术免除伤口表面炭化或深层组织烧伤，对人体无有害电磁波辐射，有效避免对神经系统的损伤。经临床使用及3~6个月的随访证实，等离子应用于慢性肥厚性鼻炎、变应性鼻炎治疗效果好，反应轻，方法简捷，不需住院，值得临床推广。

【Abstract】 Objective To discuss the clinical effect of plasma on chronic hypetrophic rhinitis and allergic rhinitis.Methods In the people who came to our hospital from January to August this year，there were 899 cases of chronic hypertrophic rhinits and 513 allergic rhinitis.Results Through the telephone follow-up after therapy and the records of returnning visits,620 cases of chronic hypertrophicrhinitis patients recovered，242 partly relieved，37 had no change，the curative rate was 95.88%;and in 513 allergic rhinitis patients，325 recovered，160 partly relieved，28 had no change，the curative rate was 95.54%.Conclusion It is not a long time for the technology of plasma to be used in clinical treatment.It is different from the microwave，high-frequent electrotome and laser optical fibre because it uses ultra-low-temperature techmique to avoid the wound charring and inner tissue burning by use of harmless electromagnetic radiation to avoid harming the nevous system efficiently.Plasma may be used for chronic hypentrophic rhinitis，allergic rhinits with good therapeutic results and mild reaction no need of inhospital.It will be spread in clinic widely.

对2005年1~8月来我院就医，明确诊断为慢性肥厚性鼻炎、变应性鼻炎［1］患者，采用湘械注标AHOS001-2001长沙市奥世计算机设备厂生产的DNR等离子治疗仪治疗患者共计1412例，回顾分析如下。
1 资料与方法
1.1 一般资料 慢性肥厚性鼻炎899例，男581例，女318例，男女比例为1.8∶1；年龄0~10岁29例，10~20岁257例，20~30岁147例，30~40岁238例，40~50岁109例，50~60岁81例，60~70岁34例，70~80岁4例。变应性鼻炎513例，男348例，女165例，男女比例2.1∶1；年龄0~10岁16例，10~20岁108例，20~30岁104例，30~40岁146例，40~50岁96例，50~60岁35例，60~70岁6例，70~80岁2例。
1.2 治疗方法 采用长沙市奥世计算机设备厂生产的DNR（湘械注标AHOS001-2001）数字热能治疗系统（治疗仪），采用数字化温控等离子低温消融技术。精确控制在靶组织表面。
1.2.1 慢性肥厚性鼻炎 常规消毒，1%地卡因棉片 1‰肾上腺素2~3滴，做鼻甲表面麻醉，术前嘱患者练习嘴吸气，鼻出气，以免将治疗中产生的少许气体吸入肺部而产生不适感，能量调至0.1~0.45左右，将探头从后往前逐步凝固下鼻甲，每点1~2s，黏膜变白即可。术后可能因鼻腔分泌物过多而阻塞鼻腔，可由医生慢慢取出，局部滴药，口服或静注抗生素，创面20~30天左右痊愈。
